To take advantage of this insurance offer

  1. Download the application form;
  2. Select the option which corresponds to your family’s needs;
  3. Return your completed form, at cpa@vigilis.ca;
  4. You will receive, by email within 5 business days, a premium statement indicating the premium required according to the chosen option;
  5. Pay the annual or monthly premium within 90 days of your child’s birth.

* Eligible Spouse: The member’s legal or common-law spouse. A common-law spouse means a person who is living with the member in a common-law relationship. A common-law relationship is considered to exist when two persons have lived together in a conjugal relationship for at least 12 months. The amount of insurance coverage for the spouse cannot exceed that of the member.
